Baptism Certificate with Notations
Most often a recent Baptism Certificate is needed before you can be married in the Catholic church. 
Recent to the church means signed, dated and embossed with the church’s seal 6 months prior to the date of your wedding.

​Certificates will also be needed before a child prepares to receive a sacrament in a church other than the church of Baptism.

​Who can request a certificate?

• A person may request his or her own sacramental record or certificate 
• Parents of a minor, one with legal guardianship of another
• A pastoral minister at another Catholic parish, or a diocesan official ​

In order to protect the confidentiality of these records, St. Ann's Church can only mail certificates directly to a parish or other church authority, or to the person named on the certificate, the parent, or legal guardian of a minor child.
Proof of identity must be provided via government-issued photo identification before the record will be released.
