Baptism Certificate with Notations
Most often a recent Baptism Certificate is needed before you can be married in the Catholic church.
Recent to the church means signed, dated and embossed with the church’s seal 6 months prior to the date of your wedding.
Certificates will also be needed before a child prepares to receive a sacrament in a church other than the church of Baptism.
Who can request a certificate?
In order to protect the confidentiality of these records, St. Ann's Church can only mail directly to a parish or other church authority.

- A person may request his or her own sacramental record or certificate
- Parents of a minor, one with legal guardianship of another
- A pastoral minister at another Catholic parish, or a diocesan official
- The spouse or adult child may request the record, if the person is deceased.
